What You'll Do
- Develop and implement personalized treatment plans with clear, achievable goals.
- Assess patient needs and adjust therapy interventions to maximize outcomes.
- Collaborate with a multidisciplinary team to ensure comprehensive care.
- Document patient progress in compliance with state and federal regulations.
- Educate patients and families on therapy techniques and home programs.
- Optimize patient functionality through screenings and targeted interventions.
- Communicate effectively with patients, families, and healthcare professionals.
- Maintain punctuality and reliability for scheduled shifts.

Requirements
Education: Bachelor's, Master's or Doctorate Degree from a college or university with an accredited Physical Therapy program.
Licenses/Certification: Current Physical Therapist license in the state employed. Current CPR certification.
Experience: Six months experience in a long-term care environment preferred.
