Las Palmas Medical Center in El Paso, Texas is seeking a dedicated Registered Nurse Transplant Coordinator to join our Healthcare & Medical Services team. As a key clinical resource within our transplant program, you will coordinate and deliver high-quality nursing care to transplant candidates, recipients, and donors, while supporting our mission of compassionate, community-focused healthcare. In this role, you will guide patients and families through the complex transplant journey, from initial evaluation through surgery, discharge, and long-term follow-up. You will perform thorough assessments, develop and implement individualized care plans, monitor patients for complications, and collaborate closely with surgeons, physicians, and an interdisciplinary team to ensure safe, effective, and timely care. Drawing on strong clinical judgment and the ability to function in high-intensity environments, you will respond to urgent and emergent needs, support stabilization efforts, and escalate care as appropriate. You will educate patients and families on transplant procedures, immunosuppressive therapy, infection prevention, lifestyle modifications, and long-term self-management, serving as a consistent point of contact and advocate throughout the process. As a liaison between patients, families, the transplant team, organ procurement organizations, and ancillary services, you will coordinate testing, procedures, follow-up appointments, and transitions of care. You will maintain accurate documentation in the electronic health record, support compliance with regulatory and accreditation standards, and participate in quality improvement, data collection, and outcomes monitoring for the transplant program. Working in an on-site clinical setting, you will be part of a collaborative culture that values teamwork, communication, and compassionate care tailored to the diverse needs of the El Paso community.
