Comfortable and knowledgeable with Point of Service (P O S) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring ( RTM) with discharged patients to home or next level of care.Team Collaboration: Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other rehab staff.Physical Therapist Assistant, PTA Qualifications:State licensure as a Physical Therapy Assistant.EMR and MS office applications.CPR/BLS certification.Knowledge of Section GG data collection and documentation requirements.
