Description: The RN implements professional nursing practice centered on the patient as a whole person and the creation of a caring environment focused on healing and wellness. Nursing practices are based on evidence, current research, as well as hospital and department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Coordinating, ensuring, and providing direct patient care activities on the unit is central to this role. This includes provision of nursing rounds, assessments, and treatments; facilitating admission, transfer, and discharge activities; provision of patient and family-centered education; care coordination and individualized treatment planning as applicable. The RN performs and documents a comprehensive assessment in accordance with the RN's scope of practice and hospital policies and procedures. The RN considers the patient's culture, age, and developmental level. The RN identifies patient risk factors and presenting complaints. Obtaining patient health information from family or others is necessary. The RN collects objective and subjective data. Verifying the initial admission assessment is essential if it is obtained by other members of the healthcare team. The RN develops a problem list, prioritizing patient needs based on input from the inter-professional team as appropriate. Actions and interventions are designed relevant to the patient’s culture and age. Desired outcomes are determined in coordination with patient and family in accordance with the medical plan of care. The RN fully implements patient care therapies and procedures with empowerment and clinical autonomy within the scope of practice in accordance with established policies, procedures, professional guidelines, culture, and age of the patient served. The RN explains procedures, treatments, and processes to patients and families. Initiating actions to alleviate patient stress and discomfort is part of the role. Recognizing, responding to, and communicating changes in patient condition appropriately is critical. The RN conducts and documents ongoing assessment and response to procedures and therapies using the nursing process according to hospital policy, established standards, culture, and age of the patient served. Tasks are delegated appropriately, and the RN oversees team members in the performance of activities regarding patient care. The RN monitors the accuracy and completeness of delegated patient care provided by other team members in the implementation of the plan of care. Identifying and addressing educational needs of patients related to prioritized problems according to level of comprehension is necessary. Facilitating the patient discharge process by executing discharge orders in a timely manner is a responsibility. Assuring the performance of discharge teaching and coordination of the plan with case management and others as indicated is required. The RN provides input into patient/nurse assignments considering available staffing, unit activity, and patient acuity. Demonstrating active participation in unit-based activities in support of organizational values and priorities is important. Promoting a positive group dynamic and healing environment through words and actions is expected. The RN assures nursing excellence in accordance with accepted standards, evidence-based practice, current research, and caring philosophy. Active participation in department and unit quality improvement activities and core measures is essential. The RN uses a holistic perspective regarding patient care.
