What You Will Do The Home Care Occupational Therapist coordinates and directs the patient’s care in the home environment based on individualized needs. Responsibilities include: Managing a caseload of home‑care patients, evaluating appropriateness of home‑care needs, and developing care plans in collaboration with providers, caregivers, and Home Care IDG members. Providing follow‑up, evaluating effectiveness of the plan, and monitoring patient and caregiver response. Delivering patient care according to provider orders, departmental policies, regulatory guidelines, and best practice. Documenting in the medical record in a timely manner per agency policy. Participating in case conferences and team meetings. Performing on‑call responsibilities. Supervising and delegating appropriate functions to Certified Occupational Therapist Assistants. Required Education Bachelor’s Degree in Occupational Therapy Preferred Education Master’s or Doctorate Degree in Occupational Therapy Required Skills Therapy skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional verbal and written communication skills. Professional interpersonal skills and ability to multi‑task. Basic organizational and time‑management skills. Basic computer skills and knowledge of general office practices. Experience working with patients and caregivers dealing with acute and chronic illness. Ability to prioritize and handle multiple critical tasks simultaneously. Preferred Skills Home Care Home Base experience. Experience with smartphones and smart devices. Required Licensure / Registration / Certification Current Texas license as an Occupational Therapist. CPR certification. Current driver’s license, auto insurance and reliable transportation. Preferred Licensure / Registration / Certification Advanced or specialty certifications (e.g., NDT, geriatric, orthopedics, lymphedema). Required Experience Work or educational experience in the healthcare field. Preferred Experience One year experience as an Occupational Therapist in a healthcare setting.
