Key Responsibilities Provide independent, comprehensive clinical care for up to 450 patients, managing acute and chronic illnesses typical of older adults. Take full accountability for patient outcomes, seeking specialist consultation when needed while remaining actively involved in detailed patient care. Coordinate care with hospitalists, transitional‑care teams, case managers, and other resources for patients in hospitals, SNFs, LTC facilities, or homebound. Lead a care team composed of a medical assistant, care facilitator, and care coordinator for office‑based patients. Serve as a health coach, helping patients set short‑ and long‑term health goals, partner on goal‑setting, and frequently follow up to promote behavior change. Simplify and prioritize care plans so patients can act on them and so care teams can meet performance goals. Maintain a panel of patients with an average of 18–21 appointment slots per day (new patients count for2 slots, follow‑ups for1 slot). Work with management and medical directors to ensure compliance with guidelines, participate in risk and quality management programs, clinical meetings, and other sessions that advance patient health and company goals. Adhere to departmental goals, standards of performance, regulatory compliance, and quality patient‑care policies and procedures. Assist in recruiting patients for the center and recruiting additional providers for the company as needed. Cover for other providers when necessary and perform other duties as assigned by management. Qualifications & Competencies Strong patient accessibility: open to walk‑in visits, respond to calls and messages promptly, and build trust through availability. Service orientation: deliver care as you would want for a family member or yourself. Evidence‑based medicine: keep current with clinical, nutritional, and lifestyle‑based interventions that improve outcomes. Leadership: continuously develop and improve leadership skills for the benefit of patients, the team, the center, and the company. Quality focus: work with care teams to achieve high‑quality outcomes and quality measures, and seek systematic improvements to practice and center operations. Influence: competently and compassionately influence patients, teams, and oneself to achieve the best outcomes. Self‑care: maintain physical, mental, social, and spiritual health to provide the best care for others. Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other relevant software. Willingness to travel locally as needed, and nationally for training and recruiting opportunities. Fluency in English (verbal and written); fluency in additional languages may be required at specific centers. Use of independent judgment and decision making. Pay Range $111,140 – $158,771 (base annual salary, subject to experience, education, and location; potential bonuses or commissions).
