Home Health OT - Greenville, NC

As a Home Health Occupational Therapist, you will:

• Confer with physician and clinical team members to obtain patient information and help develop, implement and revise the therapy treatment program related to daily living, work-related skills, community reintegration, adaptive equipment needs, strength, coordination, range-of-motion, etc
• Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient‘s needs
• Train staff and family members to manage the retraining program including dressing, feeding, grooming and hygiene skills, participate in team and family educational meetings
• Submit evaluation, treatment plans, progress reports and discharge summary to the supervisor and care management staff
• Provide Occupational Therapy Assistants and Home Health Aide staff with written instructions and care plan that reflects current plan of care as related to therapy
• Oversee Occupational Therapy Assistant and Home Health Aide staff performance, including monitoring the appropriate completion of documentation

Required Qualifications

• Current and unrestricted Occupational Therapy license in the state of North Carolina
• A valid driver‘s license, auto insurance, and reliable transportation
• Current CPR certification

• Home Health experience

Pay Range

$54.00 - $76.00 pay per visit/unit

$85,400 - $117,500 per year base pay

Additional Information

Driving Statement

You will be a part of Humana‘s driver safety program and therefore will need a valid state driver‘s license and proof of personal vehicle liability insurance with at least 100/300/100 limits.

TB Statement

You will be patient facing, therefore apart of Humana‘s Tuberculosis (TB) screening program. If selected for this role, we will require you to undergo TB screening.
