PRN Registered Nurse (RN)

As we continue to grow, we are seeking a dependable and compassionate RN to join our team and assist with in-home assessments and reassessments. This flexible PRN role is ideal for an RN looking for supplemental hours while making a meaningful impact in the community. The nurse will complete in-home visits, client assessments, and reassessments while collaborating with our care team to ensure exceptional client support and continuity of care.

Responsibilities:

• Conduct in-home client assessments and reassessments
• Complete nursing visits and required documentation accurately and timely
• Observe and report changes in client condition or care needs
• Collaborate with office staff and caregivers to support quality care plans
• Provide compassionate, professional communication with clients and families
• Assist with additional nursing support as needed

Schedule:

• PRN / Part-Time
• Estimated 2-12 hours per week with opportunity to grow
• Flexible scheduling based on client needs and availability

Qualifications:

• Active RN license required
• Previous home health, hospice, or healthcare experience preferred
• Experience working with Medicaid clients is a plus
• Strong bedside manner and communication skills
• Tech-savvy and comfortable using digital documentation systems
• Self-motivated with a flexible, "go-getter" attitude
• Reliable transportation for local visits

Compensation: $120.00 per day
