Case Management Specialist

$21.18 - $21.8 per hour

Job Details Location: FESCO Les Marquis - Hayward, CA 94541 Position Type: Full Time Education Level: High School Salary Range: $21.18 - $21.80 per hour Travel Percentage: Up to 25% Shift: Any Accommodations Except in the case of an undue hardship, La Familia provides reasonable accommodations to persons with disabilities. Position Overview The Case Management Specialist supports residents at Les Marquis House, helping them move toward self‑sufficiency and secure a permanent home. The role requires a high level of customer service, operational task execution on site, ensuring safety and security, carrying out program guidelines, handling crisis situations and assisting residents as needed. The position is covered under the SEIU 1021 Collective Bargaining Agreement and offers a 3% base pay increase in January 2027. Work hours are Monday/Tuesday 8AM–4PM and Wednesday/Thursday 4PM–12AM, with variations based on program needs. Responsibilities Case Management – Screen applicants via phone interviews; record documentation; refer to Program Manager as appropriate. Conduct intake interviews; complete intake/exit and HMIS documentation; Research and provide referral sources for residents; Assist residents in achieving weekly Action Plan goals; collect documentation; record daily plans and activities in case notes; Respond to resident requests; provide guidance and direction to residents in support of their action plans; Complete follow‑up forms for former resident calls; Maintain resource binder and Master Log binder with updated materials and forms; document case management services and complete service log. Supervise Facility and Resident House Activities – Enter data into logs and records; handle crisis situations such as illnesses or emergencies; Monitor resident sign‑ins/outs and meal logs; Receive, sort, store/dispense donated items; complete donation paperwork; Perform building security and cleanliness checks; inspect common areas, doors, windows, alarms; Inspect and sign off on resident chores; carry out facility guidelines and schedule; accomplish daily shift checklist; provide child‑care with volunteers and Activity Leaders when parents attend life‑skills classes. Food Services – Maintain kitchen area per shift checklist; support preparation of breakfast/dinner for 20+ residents; follow FESCO guidelines on cooking healthy meals; check off meals in the roster and ensure residents sign up for meals and saved plates. HMIS Data Input – Enter data from HMIS Intake and Exit Forms for all residents. Other – Facilitate or lead life‑skills workshops for residents; Attend trainings and events relevant to key responsibilities; Track and report regularly on key metrics for responsible function areas; Lead and collaborate on special projects related to agency objectives. Timekeeping & Attendance – Accurately record work hours, take required meal breaks, submit timesheets by deadlines, report scheduling conflicts, maintain reliable attendance, follow policies on overtime and leave. Working Conditions – Engages in routine sitting, walking, talking, keyboard usage, stooping, kneeling, crouching, twisting, crawling, reaching, and occasional travel; works under deadlines; noise level low to moderate. Physical Requirements – Safely operate a motor vehicle; push/pull and lift up to 50 pounds frequently; stand and move daily; sit for prolonged periods; visually focus near and far; maintain punctuality and task performance; grasp and carry items; hear and communicate effectively; identify, resolve workplace issues; process information for organization.
