Service Coordinator

Conduct assessments to identify members’ physical health, behavioral health, social services, and long-term services and supports needs Identify members at high risk for complications and those who may benefit from alternative levels of care or waiver programs Obtain clinical data as directed by the responsible RN or care coordinator Coordinate care for members with chronic illnesses, co-morbidities, and disabilities Collaborate with the interdisciplinary team to develop and implement person-centered service plans Participate in team rounds Collaborate with care coordinators to ensure effective care coordination Travel to members’ homes and other locations to conduct assessments and provide support Focus on clinical assessments and support for members’ behavioral health needs Requirements Current, unrestricted RN, LSW, LCSW, or LMHC license in applicable state(s) required Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role; or any combination of education and experience providing an equivalent background State-specified certification based on state law and/or contract may be required Strong communication and interpersonal skills Ability to work effectively as part of an interdisciplinary team Compassionate and patient-centered approach to care Behavioral health training may be required to meet contractual workforce requirements 50% travel required Core Competencies Demonstrates expertise in conducting clinical assessments and coordinating care for individuals with chronic illnesses and disabilities, while effectively collaborating with interdisciplinary teams to develop person-centered service plans.
