Key Responsibilities\n

  • Deliver high-quality therapeutic services to clients, ensuring that each interaction is respectful, empathetic, and focused on the client’s overall well-being.

  • Collaborate with interdisciplinary teams to develop comprehensive care plans, integrating psychosocial support into the hospice service delivery model.

  • Utilize innovative and evidence-based therapeutic techniques to assist clients in achieving their therapeutic goals.

  • Prepare and submit thorough documentation for each client session, ensuring compliance with company protocols and timelines to maintain the integrity of client records.

  • Conduct a minimum of 25 client-facing engagements per week, providing compassionate care while supporting a sustainable workload to promote personal well-being.

The primary focus of this role is to serve individuals and families receiving hospice care, addressing psychological and emotional needs during a profoundly sensitive period. Qualifications\n

  • A Master’s degree in Social Work (MSW) from an accredited institution.

  • Active California licensure as a Licensed Clinical Social Worker (LCSW).

  • Timeliness and organization in documenting services, complying with company standards and industry regulations.

  • Familiarity with diverse client populations, demonstrating cultural competence in treatment approaches.

  • Proficiency in Microsoft Office 365 and Electronic Health Record (EHR) systems for effective documentation and case management.

Compensation and Benefits\n
