LPN: Resident Care & Clinical Lead

Outcome HC seeks an experienced Licensed Practical Nurse (LPN) to manage unit-based care under RN supervision. You will assist with assessments, medication distribution, and clinical documentation, while supervising CNAs and other staff. Responsibilities include following Plan of Care, using SBAR communications, and ensuring infection control and resident rights are observed. Applicants must be NJ-licensed LPNs with current CPR.
