Job Details

Conduct individual nutrition assessments and create care plans
Provide diabetes education and evaluate glucose logs for medication adjustments
Develop culturally appropriate nutrition education programs
Collaborate with health education staff and care teams
Document nutritional care and maintain compliance with program guidelines

Requirements
Bachelor's degree in nutrition or dietetics (Master's preferred)
Registered Dietitian license and CDR certification required
Certified Diabetes Educator certificate preferred
One year of RD experience preferred
BLS certification required upon hire
