Job Title: Social Worker

Client: HHSC - Kohala Hospital

Location:54-383 Hospital Road Kapaau, HI 96755

Duration: 13 weeks
Pay Rate: $60/HR on W2

Schedule: Days

Key Responsibilities

• Assess clients’ social, emotional, and practical needs.
• Provide counseling, emotional support, and guidance.
• Develop and implement individual support or care plans.
• Connect clients with healthcare, education, housing, employment, and welfare services.
• Advocate for clients’ rights and access to services.
• Conduct home visits and community outreach when required.
• Maintain accurate and confidential case records.
• Coordinate with government agencies, NGOs, healthcare professionals, schools, and other organizations.
• Monitor clients’ progress and follow up on their needs.
• Participate in awareness programs and community development activities.
• Identify vulnerable individuals or families and help them access appropriate assistance.
• Prepare reports and documentation as required.

Qualifications

Bachelor's or master's degree in social work (BSW/MSW), sociology, psychology, or a related field, depending on the position.

Good communication and interpersonal skills.

Empathy, patience, and a non-judgmental approach.

Ability to maintain confidentiality.

Strong problem-solving and organizational skills.

Ability to work independently and as part of a multidisciplinary team.

Knowledge of relevant social welfare programs and community resources is an advantage.
