Family Care-Registered Nurse

Job Overview Position Title: Family Care-Registered Nurse (LTSS Service Coordinator – RN Clinician) Hiring near: Milwaukee County. Field: This field‑based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. Alternate locations may be considered if candidates reside within a commuting distance from an office. Responsibilities Perform telephonic and face‑to‑face functional assessments to identify, evaluate, coordinate, and manage member needs. Identify members at high risk for complications and coordinate care in conjunction with the member and the health‑care team. Manage members with chronic illnesses, co‑morbidities, and/or disabilities to ensure cost‑effective and efficient utilization of health benefits. Obtain a thorough member history to develop an individualized care plan. Establish short‑ and long‑term goals in collaboration with the member, caregivers, family, natural supports, and physicians; identify members who would benefit from alternative levels of care or waiver programs. Develop and oversee the care plan and ensure the member’s access to services. Assist with the implementation of care plans by facilitating authorizations and referrals for utilization of services as appropriate. Interface with Medical Directors, Physician Advisors, and/or interdisciplinary teams to manage person‑centered care plans. Assist in problem solving with providers, claims, or service issues. Minimum Requirements Associate of Science in Nursing or Bachelor of Science in Nursing. Minimum 3 years of experience in a Service Coordinator, Case Management, or similar role working with individuals with chronic illnesses, co‑morbidities, and/or disabilities. Current, active, valid, and unrestricted RN license in Wisconsin. Minimum 1 year of experience working with at least one of the Family Care target populations (older adults or individuals with disabilities). Preferred Skills and Experience BA/BS in Health/Nursing. Case management experience with older adults or individuals with disabilities. Long‑term care background. Commitment to member‑centered care and cultural responsiveness. Family Care/Partnership experience. Comfortable using technology. Strong interpersonal and communication skills. Ability to work collaboratively in a team environment. Problem‑solving and crisis‑management abilities. Willingness to travel to work sites and other locations, including members’ homes and communities for in‑person visits. Travel Requirements Travel to client sites, members’ homes, and other locations as required for in‑person assessments and care coordination. Job Level and Family Job Level: Non‑Management.
