Responsibilities
- Conduct comprehensive physical therapy evaluations for residents to identify impairments in mobility, balance, strength, gait, and functional performance.
- Develop and implement individualized treatment plans focused on restoring function, improving mobility, and reducing pain, using evidence-based practice.
- Provide direct patient care including therapeutic exercise, gait training, transfer training, neuromuscular re-education, and manual techniques as appropriate.
- Re-evaluate residents regularly, document progress, adjust treatment plans, and communicate outcomes to the interdisciplinary team.
- Educate residents and families on home exercise programs, safety strategies, and care goals; document outcomes and patient response.

Requirements
- Ability to work in a skilled nursing setting with older adults and complex comorbidities.
- Strong documentation, communication, and collaboration skills to coordinate care with nursing and medical staff.
- Demonstrated ability to manage multiple patients and maintain quality care within a travel assignment.
- Flexible and dependable, with capacity to meet the shift schedule and contract duration.
- Commitment to patient-centered care and adherence to facility policies and infection control practices.
