This role is responsible for managing a patient caseload, coordinating care with the interdisciplinary team, conducting patient assessments, and ensuring patients and their families receive the highest quality hospice services. The ideal candidate is a skilled clinician with strong communication abilities and a passion for delivering patient-centered care. Schedule & Work Details Coverage Area: Within 50 miles of Springfield, MO Schedule: Monday through Friday, 8:00 AM to 4:30 PM Hours: 8 hours per day, 5 days per week Pay Rate: $51.00 - $53.00 per hour Weekday On-Call: 4:30 PM to 8:00 AM, 3 days per week Weekend On-Call: Saturday 12:00 AM through Monday 8:00 AM, 2 weekends per month Mileage Reimbursement: Yes, reimbursed between patient visits Key Responsibilities Serve as the primary RN Case Manager for an assigned hospice patient caseload. Provide skilled nursing care and symptom management for hospice patients in home and residential settings. Perform comprehensive patient assessments and ongoing evaluations of physical, emotional, and psychosocial needs. Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team. Coordinate patient care with social workers, chaplains, hospice aides, physicians, and other healthcare professionals. Participate in Interdisciplinary Group (IDG) meetings every other Wednesday to review patient progress and care plans. Educate patients and family members regarding disease progression, symptom management, medications, and end-of-life care. Maintain accurate, timely, and compliant clinical documentation. Respond to patient and family needs during assigned on-call rotations. Advocate for patient comfort, dignity, and quality of life throughout the hospice journey. Required Qualifications Active Registered Nurse (RN) license. Current Basic Life Support (BLS) certification. Strong clinical assessment and patient care skills. Excellent communication, interpersonal, and compassionate bedside manner. Valid driver's license, reliable transportation, and current auto insurance. Ability to manage a field-based caseload and travel throughout the assigned territory. Preferred Qualifications Previous hospice nursing experience. Experience using Homecare Homebase (HCHB) documentation software. Case management experience in hospice, home health, or palliative care settings. Strong organizational and time-management skills. Ability to collaborate effectively within an interdisciplinary care team. Dress Code No specific dress code or scrub color requirements.
