YOUR ROLE CLINICAL LEAD IN THE HOME

You are the clinical anchor for home-based care.

Conduct in-home patient assessments and ongoing evaluations
Develop, update, and oversee individualized care plans
Supervise and support CNAs, GNAs, HHAs, and Med Techs
Monitor changes in patient condition and coordinate follow-up care
Communicate with families, physicians, and care teams
Ensure safe, compliant, high-quality care delivery
Maintain accurate and timely clinical documentation
Provide guidance, education, and clinical support to caregivers

WHAT WERE LOOKING FOR

Active Maryland RN license (or compact license eligible in MD)
CPR/First Aid certification
Reliable transportation for home visits
Strong clinical assessment and communication skills
Experience in home care, hospice, or geriatrics preferred
