Qualifications

• Unrestricted license to practice dentistry in the Commonwealth of Virginia
• DEA license to prescribe medication
• Board certification in specialty area (if applicable)
• Current Basic Life Support (BLS) certification with hands-on practicum
• Must not be excluded or sanctioned under Medicare or Medicaid
