Licensed Master Social Worker

Human Touch Home Health is hiring a compassionate Licensed Master Social Worker (LMSW) to provide medical social work services in Falls Church, Arlington, Alexandria, Fairfax, and surrounding Northern Virginia communities.

What You'll Do

• Assess patients' social, emotional, and financial needs
• Connect patients and families with community resources and support services
• Provide counseling, crisis intervention, and discharge planning
• Coordinate care with nurses, therapists, physicians, and the interdisciplinary team
• Complete documentation accurately and on time

Requirements

• Active Virginia Licensed Master Social Worker (LMSW) license
• Valid driver's license and reliable transportation

• Home health or healthcare experience preferred
