Community Bridges

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona, including residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services for individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Lead Navigator A Lead Navigator is an individual who is in recovery from AOD use and/or GMH for at least one year and has at least 90 days experience as a Navigator or Peer Support Specialist. The primary role involves working with homeless individuals, providing outreach and supportive services to assist with ending homelessness. The Lead Navigator serves as the bridge for the patient, coordinating and monitoring RBHA and non‑RBHA community‑based services and facilitating delivery of these services. The Lead Navigator seeks out individuals with the highest acuity and greatest vulnerabilities, conducting outreach, engagement, case management, and ongoing wrap‑around supportive services. The Lead Navigator mentors Navigator II staff, helping them onboard, understand the program, and fulfill program requirements. The Lead Navigator masters coordinated entry, housing referral, leasing, wrap‑around services, and discharge processes, working as part of a multidisciplinary team that includes patient care techs, registered nurses, counselors, and clinical leads. Skills/Requirements High school diploma or GED is required. Associate’s degree in a field related to behavioral health is preferred. 1–3 years of full‑time health‑care related experience, or equivalent education and experience. Current Arizona driver’s license. Clear 39‑month motor‑vehicle record. Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9‑10‑101.33 and CBI clinical policies and procedures. Arizona fingerprint clearance card, site specific.
