Nurse Practitioner (Primary Care - Assisted Living)

Job Type: Full-Time (Part-Time Considered for Port Jervis Only)
Location: Port Jervis & Middletown, New York
Work Setting: Assisted Living Communities/Primary Care

• Full-Time: 30 hours per week

• Four-day work week

• Typical shifts are 4 hours and scheduled Monday-Friday

• Preferred shift times:

  • 9:00 AM - 1:00 PM

  • 1:00 PM - 5:00 PM

• Consistent weekly schedule expected

• No nights

• No weekends

• No on-call responsibilities

Summary

We are seeking a compassionate and experienced Nurse Practitioner or Physician to provide high-quality primary care services to residents in assisted living communities across Port Jervis and Middletown, NY. This position offers an outstanding work-life balance with no nights, weekends, or on-call responsibilities. Providers will deliver patient-centered geriatric care while working collaboratively with an interdisciplinary team to improve the health and well-being of older adults.

Responsibilities:

• Perform comprehensive health assessments, physical examinations, and medication reviews.

• Develop individualized care plans in collaboration with the interdisciplinary care team.

• Diagnose and manage acute and chronic medical conditions.

• Order, interpret, and review laboratory tests and diagnostic studies.

• Prescribe medications and treatments within the scope of practice.

• Educate residents and families on disease prevention, health maintenance, and chronic disease management.

• Maintain accurate and timely documentation using eClinicalWorks (eCW).

• Participate in quality improvement initiatives and interdisciplinary care planning.

Qualifications:

Required

• Active New York State Nurse Practitioner

• Active DEA registration.

• Currently enrolled and credentialed with both Medicare and Medicaid.

• Nurse Practitioners must have at least 3,600 clinical practice hours to practice independently under New York State regulations.

• Master's Degree in Nursing (NP candidates).

• Minimum 3 years of clinical experience.

• Primary care experience.

Preferred

• Geriatric medicine experience.

• Assisted living or long-term care experience.

Important: Candidates must already be actively enrolled and credentialed with Medicare and Medicaid. Applicants who are only eligible for enrollment or would need to begin the credentialing process cannot be considered due to current operational needs.

Compensation:

• Compensation Model: W-2 or 1099 available.

• Sign-On/Retention Bonus(Only Full Time

Part-time opportunities may be considered for providers interested in working only in Port Jervis. Full-time providers will cover both Port Jervis and Middletown.
