Professional Development – Ongoing training and growth opportunities Established Nonprofit Organization – Stability with mission-driven leadership The Case Manager is responsible for developing, writing, and implementing Individual Service Plans (ISPs), providing written periodic updates, and ensuring all waiver requirements are met for assigned participants. This role includes identifying achievable goals and objectives, developing programs within the Therap electronic records system, and monitoring implementation for quality and compliance. The Case Manager works closely with Direct Support Professionals and supervisors to ensure health, safety, and meaningful programming. Develop and implement Individual Service Plans and structured goal programs Enter and maintain programming within the Therap system Monitor goal implementation and provide periodic written updates Provide leadership and direction to Direct Support Professionals Ensure overall health, safety, and well-being of assigned participants Coordinate vocational opportunities with Community Supports Supervisor Participate in team meetings and program planning Provide crisis management and behavioral intervention when necessary Clearly communicate Parc's mission and values within the community This role requires both administrative expertise and hands-on engagement Experience working with adults with intellectual and developmental disabilities Prior case management experience Understanding of behavior management and conflict resolution techniques Ability to manage multiple priorities in a fast-paced environment High school diploma or equivalent required; Bachelor's degree preferred Minimum of 3 years' experience working with intellectually and developmentally disabled adults Minimum of 4 years' experience working with intellectually and developmentally disabled adults or a 4 year degree in social services, human services, psychology, etc. Demonstrated case management and/or vocational services experience Ability to meet physical, visual, and auditory demands of the role Must communicate effectively in English, verbally and in writing Monday through Friday, 7:30 AM – 3:30 PM. ~ This is an on-site position at Parc Center for Disabilities. 10 paid holidays (available immediately)
