Assignment Overview

This contract assignment is anticipated to last for 13 weeks and is structured as a Day 5x8 hour schedule from 8:00 AM to 5:00 PM, Monday through Friday. There will be no weekend rotation or on-call responsibilities associated with this position.

Position Details

• Region: Home Health
• Territory: Candidates will cover Fitchburg, Leominster, Lancaster, Lunenburg, Princeton, Westminster, Sterling, Clinton, Holden, Boylston, and West Boylston.
• Average Daily Caseload: 58 patients
• Weekly Productivity Expectation: 30 points
• Travel Requirements: Up to 125 miles per week, with reimbursement for mileage at the IRS standard rate.

Clinical Responsibilities

The clinician will provide home visits and participate in periodic office meetings. A typical day may involve seeing 5 to 10 patients and completing various visit types, such as:

• Start of Care visits
• Wound Care assessments
• Follow-up visits
• Other adult home health services

Minimum Qualifications

Applicants must possess the following qualifications:

• One year of recent adult home health or hospice experience
• Required certifications: CPR and BLS
• Active nursing license (pendings are not accepted)
• Proficiency in Homecare Homebase (HCHB) and OASIS within the last six months
• Reliable transportation, driver's license, and liability insurance

Expectations

• Participants are expected to accept all patients assigned to their schedule, regardless of distance.
• Documentation must be completed within 24 hours of patient appointments.
• Contract hours are not guaranteed, and clinicians must achieve the required weekly productivity points of 30, correlating to approximately 40 hours per week.
• In lower census situations, clinicians may have the opportunity to float to nearby clinics for additional hours.

Work Environment

Clinicians are expected to wear scrubs and should be prepared for daily travel within the specified territory. The position may require flexibility regarding patient assignments and travel distances.
