RN Care Manager: Discharge & Care Coordination
McLaren Health Care
McLaren Health Care is seeking a Care Management RN to coordinate proactive patient transitions across the continuum of care. You will work with physicians, nursing staff, and social services to optimize LOS and resource use, reducing readmissions and ER visits. The role requires an active Michigan RN license, a BSN or higher, and at least three years of acute hospital care experience.
