Job Summary We are seeking a compassionate and dedicated Registered Nurse (RN) Case Manager to provide high-quality home health and hospice care. In this role, you will manage and coordinate patient care, perform comprehensive assessments, develop individualized care plans, and collaborate with an interdisciplinary team to ensure the highest standard of patient-centered care in home and community settings.

In this role, you will manage and coordinate patient care, perform comprehensive assessments, develop individualized care plans, and collaborate with an interdisciplinary team to ensure the highest standard of patient-centered care in home and community settings.
Key Responsibilities
- Manage and coordinate care for home health and hospice patients.
- Perform comprehensive patient assessments and develop individualized care plans.
- Collaborate with physicians, interdisciplinary team members, patients, and families.
- Provide skilled nursing care, patient education, and family support.
- Maintain accurate and timely clinical documentation in the EMR system while ensuring compliance with regulatory standards.
- Participate in an on-call rotation, including occasional evenings and weekends.
- Deliver compassionate, patient-centered care while promoting positive clinical outcomes.
- Current and active Idaho Registered Nurse (RN) license.
- 1-2 years of nursing experience preferred; home health or hospice experience is highly preferred.
- Strong clinical assessment, organizational, and communication skills.
- Ability to work independently and effectively manage a patient caseload.
- Compassionate approach to patient care.
- Valid driver's license and reliable transportation.
- Full-Time
- Monday-Friday, 9:00 AM - 5:00 PM
- Occasional weeknight and weekend on-call responsibilities
- Starting at $30/hour or higher based on experience.
- Guaranteed 40 hours per week (hourly pay, not per visit).
