Position Highlights Full-time opportunity Practice within an ADA Certified Diabetes Center Comprehensive diabetes management across the adult population Collaborative, team-based care model Strong clinical support allowing providers to practice at the top of their license Opportunity to build long-term relationships with patients and positively impact chronic disease management Qualifications Required Licensed Nurse Practitioner or Physician Assistant eligible to practice in Minnesota Minimum of five (5) years of clinical experience in diabetes management, endocrinology, primary care, or a closely related specialty Strong clinical assessment, diagnostic, and patient education skills Commitment to evidence-based, compassionate patient care Preferred Board Certification in Advanced Diabetes Management (BC-ADM) and/or significant experience in adult diabetes management Experience in endocrinology practice Experience managing patients utilizing insulin pumps, continuous glucose monitoring (CGM), and advanced diabetes technologies Compensation and Benefits Salary range $105,000 - $160,000 based on experience.
