If so, we have an exciting opportunity for you in the heart of Sunnyvale, CA. Our healthcare partner is seeking a dedicated Registered Nurse specializing in Case Management to join their team. In this role, you will be responsible for admitting patients to hospice services, assessing their needs, and developing individualized care plans. You will work closely with a multidisciplinary team to ensure the highest quality of care for patients and their families.

As a Hospice Nurse, your responsibilities will include:

• Admitting patients to hospice services and assessing their physical, psychosocial, environmental, safety, and developmental needs
• Developing and documenting individualized care plans based on assessments
• Coordinating care with a multidisciplinary team
• Participating in the hospice program's quality assessment performance improvement program
• Conducting follow-up visits as needed
• Need 1 year experience in setting
