Job Description
Title: Acute Hospital Case Manager - Acute Care Schedule:
- Weekend requirement: At least 2 weekend shifts in a 4-week schedule.
- Holiday requirement: Must commit to working 3 of the 5 holidays. Requirements:
- California state license required (pending licenses accepted).
- 2 years of experience in acute care hospital inpatient case management.
- Knowledge of InterQual and Epic required.
- Experience in care coordination, admission reviews, and discharge planning. Roles & Responsibilities:
- Coordinate patient care and facilitate discharge planning in an acute care setting.
- Conduct admission criteria reviews and ensure compliance with insurance authorizations.
- Assess patient needs and work collaboratively with interdisciplinary teams.
- Manage a daily caseload of 20-22 patients, ensuring effective communication with patients and families. Compliance / Notes:
- Pre-approved time off: One per contract.
- Dress code: Business casual with lab coat, or dark solid scrubs with lab coat.
- LA fire card required (within the last 30 days).
