Registered Nurse - Home Health

Job Description

Job Summary:
Seeking experienced Home Health Registered Nurse to provide high-quality patient care within LA County. Must demonstrate strong clinical skills, punctuality, and compliance with all travel and certification requirements.

Responsibilities:

• Manage a caseload of 5-6 patients daily within a non-per visit, hourly pay model.
• Conduct comprehensive home health assessments, including recent OASIS documentation.
• Adhere to scheduled start times and coordinate with team members to ensure seamless patient care.
• Maintain up-to-date CA license, valid at the time of submission, and submit required references.
• Comply with travel, background, and OSHA policies, including no recent DUI offenses.
• Complete required modules and certifications via facility platforms prior to employment.

Qualifications:

• Possess at least 2 years of home health experience with high-volume case loads.
• Have a minimum of 1 year of prior home health travel experience; general travel NOT accepted.
• Demonstrate recent OASIS expertise, including passing an OASIS E11 exam.
• Hold active California RN license in good standing at submission.
• Be able to pass background checks and provide two supervisor or higher references from within the past year.
• Exhibit flexibility to float within skill set and work variable schedules.
