Your Role: Home Health Speech Language Pathologist You will provide one‑on‑one care in patients’ homes, helping patients improve communication, cognitive skills, swallowing function, and confidence in daily living. Key Responsibilities Conduct in‑home speech therapy evaluations and follow‑up visits. Develop and implement individualized treatment plans. Assess and treat speech, language, cognitive, voice, and swallowing disorders. Educate patients and caregivers on communication techniques and home exercises. Document visits using WellSky Kinnser EMR. Collaborate with physicians, RNs, and the interdisciplinary care team. Qualifications Active Arizona Speech Language Pathologist license. Master’s Degree in Speech‑Language Pathology or related accredited program. ASHA Certificate of Clinical Competence (CCC‑SLP) preferred. CFY candidates welcome to apply. CPR certification (AHA) preferred. Minimum 1 year of clinical experience (home health preferred). Strong clinical judgment and time management.
