Requirements:

• Proficient in METs testing evaluating and diagnosing respiratory and cardiovascular
• Own a laptop (no Mac/Apple products)
• AANP
• Active and unrestricted full license
• Must be willing to complete DMA training to become DMA certified
• CV needs current home address listed for presentation
• License number
• Certification Number
• Willing to perform Gen/Med: (yes or no)
• DMA certified: (yes or no)

• VES location provider is applying to
• Open to travel to other locations? If yes any limits?
