Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings to craft personalized treatment plans
• Deliver physical therapy interventions with a focus on enhancing mobility, strength, balance, and overall function
• Collaborate closely with a multidisciplinary team, including nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Provide education to patients and their families regarding therapeutic goals, discharge plans, and exercises for home care
• Maintain accurate documentation of evaluations, treatments, and patient progress per facility and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to ensure seamless and coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
