PHYSICAL THERAPIST - FULL TIME - GRANITE CREEK HEALTH AND REHABILITATION – PRESCOTT, AZ ** $10,000 SIGN ON BONUS AVAILABLE! ** Granite Creek Health and Rehabilitation in Prescott, AZ, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a full time Physical Therapist for our in-house rehab program! Key Responsibilities: Assess, evaluate, and develop treatment plans for patients with cognitive and/or ADL physical functioning disorders. Administer rehabilitation interventions including ADL retraining, therapeutic exercises, modalities, and the use of assistive devices. Regularly assess patient progress, adjusting treatments accordingly, and communicate updates with the healthcare team. Provide counseling and consultation to patients, families, caregivers, and service providers. Document treatment plans and outcomes in line with regulatory and clinical practice standards. Contribute to patient-centered care by evaluating treatment effectiveness and making necessary referrals. Qualifications: Physical Therapist license is required. Open to Physical Therapists at all experience levels. New Grads are welcome!
