Clinical Case Manager

Description SUMMARY:
The Clinical Case Manager is responsible for providing comprehensive, trauma-informed case management and behavioral health support services to individuals experiencing homelessness, mental health challenges, substance use disorders, and other barriers to housing stability and self-sufficiency. This position utilizes evidence-based practices, including Motivational Interviewing (MI) and Cognitive Behavioral Therapy (CBT) interventions, to engage participants, develop individualized treatment plans, facilitate behavioral health groups, and coordinate supportive services. The Clinical Case Manager works collaboratively with multidisciplinary teams, healthcare providers, housing partners, and community agencies to promote participant wellness, recovery, crisis stabilization, and successful housing outcomes. ESSENTIAL JOB FUNCTIONS:
• Conduct comprehensive biopsychosocial assessments, behavioral health screenings, and ongoing participant evaluations.
• Develop, implement, monitor, and update individualized treatment and service plans based on participant strengths, goals, and identified needs.
• Utilize Motivational Interviewing techniques to increase participant engagement, self-efficacy, and progress toward treatment goals.
• Apply Cognitive Behavioral Therapy (CBT)-informed interventions to support participants in developing healthy coping skills, emotional regulation, and problem-solving strategies.
• Provide crisis intervention, suicide risk assessment, safety planning, and de-escalation services as needed.
• Facilitate psychoeducational and therapeutic group sessions focused on mental health, substance use recovery, life skills, housing stability, and wellness.
• Coordinate referrals and linkage to behavioral health treatment, medical care, substance use services, employment resources, benefits assistance, and community support programs.
• Maintain regular contact with participants through individual counseling sessions, case management meetings, wellness checks, and follow-up appointments.
• Collaborate with psychiatrists, therapists, medical providers, housing specialists, and other community partners to coordinate participant care.
• Assist participants in obtaining documentation, benefits, identification, income verification, and other resources necessary for housing and treatment goals.
• Monitor participant progress toward treatment and housing objectives, documenting outcomes and adjusting service plans as necessary.
• Maintain accurate, timely, and compliant documentation in electronic health records (EHR), HMIS, and other required systems.
• Participate in multidisciplinary team meetings, case conferences, treatment planning meetings, staff trainings, and quality improvement initiatives.
• Ensure compliance with agency policies, licensing requirements, funding regulations, confidentiality standards, and professional ethical guidelines.
• Support participant transitions to permanent housing and coordinate aftercare and community-based support services.
• Maintain professional boundaries and provide services in a trauma-informed, culturally responsive, and person-centered manner. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:
• Knowledge of case management principles, housing stabilization practices, and crisis intervention strategies.
• Understanding of Housing First, trauma-informed care, harm reduction, and person-centered service delivery models.
• Knowledge of homelessness systems, supportive housing programs, community resources, and benefits systems.
• Ability to engage individuals with complex behavioral health, substance use, and social service needs.
• Strong interviewing, assessment, documentation, and organizational skills.
• Ability to manage multiple priorities, deadlines, and a dynamic caseload effectively.
• Strong verbal and written communication skills.
• Ability to de-escalate difficult situations and respond calmly in crisis environments.
• Proficiency in electronic case management systems, HMIS, Microsoft Office Suite, and database documentation.
• Ability to work independently while maintaining collaboration with multidisciplinary teams.
• Strong problem-solving and critical-thinking abilities.
• Ability to maintain participant confidentiality and exercise sound professional judgment.
• Cultural competency and ability to work effectively with diverse populations
• Ability to maintain accurate records and meet compliance timelines. REQUIRED EDUCATION AND EXPERIENCE:
• Bachelor's degree in Social Work, Human Services, Psychology, Public Health, Counseling, or a related field required; equivalent combination of education and relevant experience may be considered.
• Minimum of two (2) years of case management experience working with individuals experiencing homelessness, behavioral health challenges, substance use disorders, veterans, or other vulnerable populations preferred.
• Experience conducting participant assessments, service planning, and community resource coordination required.
• Experience with housing programs, shelter services, supportive housing, or homelessness response systems strongly preferred.
• Experience using HMIS, electronic health records, or case management databases preferred.
• Knowledge of VA-funded programs, housing navigation, or benefits systems preferred (if applicable).
