Key Responsibilities Hospital Outreach Meet with hospital discharge planners and case managers Provide information about available programs and services Maintain relationships with hospital departments. Physician Outreach Visit pediatrician and specialty physician offices Provide referral materials and program information Answer questions about services. Community Partnerships Connect with therapy providers and case management agencies Attend healthcare networking events. Referral Coordination Track incoming referrals Ensure families are contacted quickly Coordinate intake appointments. Qualifications Preferred experience in: healthcare marketing case management therapy or healthcare programs community outreach. Strong communication and relationship-building skills are important.
