Occupational Therapist

Job Summary:

The Occupational Therapist in Home Health provides comprehensive evaluation, treatment, and coordination of care for patients in their homes. This role involves assessing patients' functional abilities, delivering direct therapy services, selecting appropriate therapeutic modalities, and collaborating closely with the interdisciplinary care team. The Occupational Therapist educates patients and families on treatment plans and documents all clinical activities to ensure quality care delivery. Responsibilities:

• Screen, evaluate, and re-evaluate patients' functional abilities in a home health setting.
• Provide direct occupational therapy treatments tailored to patient needs.
• Select and apply appropriate therapeutic modalities to improve patient outcomes.
• Communicate effectively with interdisciplinary team members and patients to coordinate care.
• Document evaluations, treatment goals, objectives, and patient progress accurately.
• Educate patients and family members on the rationale and procedures of treatments.

Required Skills & Certifications:

• Licensed Occupational Therapist in Vermont and New Hampshire or hold an unencumbered compact license.
• Bachelor's degree in Occupational Therapy or completion of a postgraduate Occupational Therapy Certification program.
• Successful completion of the National Registration Exam for Occupational Therapists.
• Experience in a home health setting preferred.

Preferred Skills & Certifications:

• Prior experience working in home health or hospice care environments.
• Strong communication and interdisciplinary collaboration skills.

Special Considerations:

• Work is performed in patients' homes, requiring travel within the service area.
• Ability to work independently while maintaining close communication with the care team.
• Must adhere to home health care regulations and safety protocols.

Scheduling:

• Full-time position with day shifts.
• Visits typically last over 60 minutes, allowing for meaningful patient interaction.
• Schedule may require flexibility based on patient needs and geographic coverage.
