The State of Connecticut, Department of Developmental Services (DDS) – West Region, Private Administration – is accepting applications for two (2) Clinical Nurse Coordinator positions located in Cheshire and Waterbury, CT. We are seeking a compassionate and highly organized Clinical Nurse Coordinator to lead and support the delivery of comprehensive, person-centered healthcare services. In this role, you will coordinate clinical programs, conduct health assessments, develop and monitor individualized healthcare plans, and collaborate with interdisciplinary teams to ensure continuity of care across treatment and community settings. DDS Mission Statement The mission of the Department of Developmental Servicers is to partner with the individuals we support and their families, to support lifelong planning and to join with others to create and promote meaningful opportunities for individuals to fully participate as valued members of their communities. Purpose of the Job Class In a state facility this class is accountable for coordinating the various facets of clinical programs involving multiple units or serving as liaison between the facility and community programs and placement settings. Examples of Duties Assist the Nurse Consultant/Supervisor in the monitoring of nursing issues in various private sector agencies, LTC facilities and community settings; Follow up on individual care in community programs; assist with assuring individuals are in least restrictive environment; Act as a resource person to agencies and/or planning and support teams (PST). Act as a resource person and provide support for individuals receiving individualized home supports (IHS). Conduct individual health audits/nursing audits and site-specific audits. Perform record keeping functions and maintaining of records. Assist the Nurse Consultant and/or Nurse Supervisor with reviewing and tracking various reports (such as medication quarterlies). Provide a variety of medical monitoring as an Ominibus Reconciliation Act (OBRA) Nurse. Act on OBRA referrals for DDS individuals and others with suspected developmental disabilities seeking nursing facility admission (including new referrals, extensions, change in condition). Track short-term stays for DDS individuals. May develop, implement, and review individual health care plans. May clinically supervise direct care staff and LPNs in execution of clinical programs. Participate or chair individual centered interdisciplinary teams. May participate in and/or cover other programs and committees as assigned. Act as a resource person to staff and community. Prepare reports. Participate in regional on-call system. Knowledge, Skill and Ability Knowledge of nursing principles and clinical therapeutic models of individual care with emphasis on interdisciplinary team approaches, individual treatment planning including impact of medical insurance program requirements for reimbursement, structure of services for individuals and their families in institutional and community-oriented settings, aftercare facilities. Considerable interpersonal skills and oral and written communication skills. Minimum Qualifications – General Experience Three (3) years of professional nursing experience. Minimum Qualifications – Substitutions Allowed A Bachelor's degree in nursing may be substituted for one (1) year of the General Experience. A Master's degree in nursing may be substituted for one (1) additional year of the General Experience. Preferred Qualifications Experience working with individuals with Intellectual and/or Developmental Disabilities. Experience following current infection control practices. Experience with clinical documentation including documenting events in writing. Experience reviewing health information and formulating reports based on that information. Experience using Microsoft Office Suite including Outlook, Teams, Word, and Excel. Experience communicating, interacting with, and explaining information to internal partners, external partners, and the public. Experience working in community and family home settings and coordinating in home supports. Experience making periodic schedule adjustments based on organizational needs. Experience collaborating with various interdisciplinary team members, health care providers, clinicians, and staff. Experience and demonstrated comprehension of current DDS Medication Regulations. Special Requirements Incumbents in this class must possess and retain a license as a Registered Nurse in Connecticut OR hold a Multistate or Compact Registered Nurse license.
