Speech-Language Pathologist (SLP)

We are seeking a licensed, compassionate, and patient-focused Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis.

The SLP will provide skilled speech, language, cognitive-communication, and swallowing services to residents in accordance with physician orders and individualized care plans. This role focuses on helping residents improve communication, swallowing safety, functional independence, and overall quality of life .

Key Responsibilities

• Evaluate residents and develop individualized speech-language therapy treatment plans based on clinical needs and physician orders.
• Provide skilled speech, language, cognitive-communication, and swallowing therapy services as appropriate.
• Assess and treat communication, cognition, voice, and swallowing-related needs.
• Monitor resident progress and modify treatment approaches as clinically appropriate.
• Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
• Collaborate with nursing, therapy, dietary, medical, and interdisciplinary team members to support positive resident outcomes.
• Participate in care planning, utilization review meetings, interdisciplinary conferences, and other clinical meetings as assigned.
• Educate residents, families, and staff regarding therapy goals, communication strategies, swallowing precautions, and treatment plans.
• Recommend and implement appropriate therapeutic strategies and compensatory techniques.
• Maintain established productivity, documentation, and quality standards.
• Support regulatory compliance, reimbursement requirements, and survey readiness.
• Promote resident safety, dignity, communication, independence, and quality of life.

Qualifications

• Graduate of an accredited Speech-Language Pathology program.
• Current and active state license as a Speech-Language Pathologist.
• Current Certificate of Clinical Competence (CCC) from ASHA.
• Long-term care or skilled nursing experience preferred.
• Strong clinical documentation and communication skills.
• Excellent interpersonal and organizational skills.
• Ability to work effectively within an interdisciplinary healthcare team.

Physical Requirements

• Ability to stand, walk, bend, and assist residents throughout the workday.
• Ability to safely lift, transfer, and support residents during therapy activities, as required.
• Ability to perform physical tasks associated with patient care and therapeutic treatment.
• Comfortable working in a clinical and long-term care environment.
• Ability to manage the physical and emotional demands of patient care.
