Primary Care Opportunity in Metro Allentown, PA – Close Proximity to NYC & Philadelphia Several Options: Locums, Locums to Permanent, or Permanent. $100/hr + malpractice insurance, lodging. Schedule: Mon‑Fri, 8:00‑4:30; Call every 5th weekend. Essential Duties Provides direct primary care for an appropriate panel size of participants. Performs interval assessments on new and established participants, as required by PACE regulations. Admits participants to hospital or other healthcare facility, providing primary care attending responsibilities and/or coordinates and manages care through proactive collaboration with hospitalists; provides updates to the Interdisciplinary Team (IDT) on a regular basis and, when required, assists with the coordination of discharge planning with the IDT. Conducts home visits, as required. Provides medical leadership and guidance to the IDT to achieve care plans goals, meet emergent and acute needs of participants, and assures comprehensive and continuous care. Ensures compliance with organizational policies and procedures and all applicable regulatory requirements and standards of practice. Participates in clinical process improvement activities to ensure the eLIFE programs attains the highest quality outcomes for its participants and satisfies all PACE regulatory requirements. In conjunction with the Medical Director, provides medical expertise and consultation regarding the design, implementation and evaluation of clinical services; including creation of care protocols, pathways or tools and monitoring practices that ensure the appropriate utilization of medical and institutional services to achieve desired quality and fiscal outcomes. Participates in the medical on‑call rotation to ensure medical coverage for eLIFE participants after hours and on weekends. Requirements Licensed to practice medicine within the Commonwealth of Pennsylvania. Board Certified in Internal Medicine or Family Medicine and Geriatrics.
