A professional opportunity is available for a Home Infusion Staff Pharmacist based in Syracuse, NY. This direct hire role offers a Monday through Thursday schedule, from 8:00 AM to 5:00 PM, supporting a dynamic healthcare team.

Key Responsibilities:

• Review infusion orders and patient therapies to ensure accuracy and safety.
• Participate in clinical rounds, collaborating with healthcare teams to optimize patient care.
• Perform sterile compounding and review total parenteral nutrition (TPN) orders.
• Maintain detailed patient records and individualized care plans.
• Ensure compliance with USP standards and cleanroom protocols.
• Train and support pharmacy staff to maintain high-quality service delivery.
• Identify and drive operational improvements within the home infusion department.

Qualifications and Experience:

• Current licensure as a Registered Pharmacist in New York State is required.
• Preferred minimum of two years’ recent experience in an acute care setting or at least one year of recent experience in home infusion therapy.
• Strong knowledge and hands-on experience with intravenous admixture services and intravenous therapy.
• Demonstrated ability to evaluate and monitor drug therapy in collaboration with physicians, nurses, and other healthcare professionals, tailored to the unique needs of home care patients.
