STATUS / FLSA Full Time – Exempt

Des: REPORTS TO: Chief Operations Officer

CERTIFICATION: DEGREE: Master’s Degree in Social Services , or social work or related field is required.

EXPERIENCE: required. Two (2) years supervisory or administrative experience and two (2) years’ experience working in Care Coordination or Case Management Services

AGENCY REQUIREMENTS:

• Valid NYS Driver’s License

• Use of personal vehicle during working hours

• Clearance of the NYS Justice Center Requirements: Staff Exclusion List (SEL), NYS State Central Registry for Child Abuse and Maltreatment (SCR), Criminal Background Check (CBC) 

SK I LLS / QUALIFICATION S :

Ability to read, understand and apply a variety of correspondence, law material, reports, legal documents and proposed and actual legislation and regulations

Attention to detail is critical in this position

Ability to analyze and interpret moderate to complex information and/or data on a frequent basis

Ability to work in a fast-pace d , collaborative environment with minimal supervision

Project coordination skills, including business process modeling, strong follow-up, meeting facilitation a n d risk assessment/mitigation

Excellent verbal and written communication skills, including by phone, email, correspondence and documentation

Ability to create and present effective and persuasive public presentations

Working knowledge of Microsoft Office application s with intermediate proficiency , to include the ability to set up spreadsheets, create formulas, and manage databases

Effective communication skills at both an individual and group level

Demonstrate knowledge of and Passion for the Pinnacle Community Services Mission, Vision, Values.

Complies with all policies and procedures and all governmental and legal requirements

JOB RESPONSIBLITIES :

• Supports and oversees all Medicaid Health Home and Non-Medicaid Niagara County Care Coordination Services and Programs, this includes intake and referrals, service operations, supervision of services and the assurance of quality service provision, member standards of outcome and funder requirements are accurate and complete.

• Develops and Oversees internal systems to ensure program service standards and efficient/accountability in program documentation billing/expense and operations oversight methods to ensure adequate income generation to cover the cost of service delivery.

• Sets and monitors program standards in cooperation with the Chief Operations Officer and Chief Administrations Officer to ensure best practice models are implemented in service provision.

• Provides and ensures the ongoing provision of staff supervision and training of all staff within the department. Provides direct or indirect staff training as needs are identified and ensures opportunities for staff development in external training settings based on professional and performance goal setting .

• Provides advocacy for organization , the community members and the well being of services by representing the agency at all assigned provider and coalition meetings.

• Established positive working relationships with the Health Homes, the Niagara County LGU/DSS funders, referral services and other providers, potential collaborative provider organizations, the community at large and other related entities.

• Identifies funding and service gaps and collaborates with the agency administration to proactively modify spending, identify new funding opportunities, identify and develop new service models to meet those needs .

• Ensures that all Care Coordination Department services meet all Federal, State, County and local regulations as well as meeting the needs of the members serve d and their families.

• Serves as a member of the Agency Management Team (Daring Discussions Group) and supports agency initiatives.

• Ensures a positive team building experience for Care Coordination Team members, supports agency wide team building.

• Assists in promoting the Agency Advancement and Communication/Marketing activities, including promoti ng positive storytelling in a trauma informed manner while preserving member confidentiality.

• Regularly attends professional development and supervisory meetings with the Chief Operations Officer.

• Performs other duties as assigned to uphold the highest standards of service and support for individuals in the community and to ensure a positive work environment for all members of the Pinnacle Community Services Team.
