Outreach Case Manager & Community Health Worker

PDX Saints Love

PDX Saints Love is seeking an Outreach Case Manager / Community Health Worker to provide street outreach, resource navigation, shelter and housing support, and intensive case management to people experiencing homelessness in Multnomah County.

You will bridge clients to shelter, healthcare, IDs, benefits, and services, while documenting activities and coordinating with partners. Candidates should be compassionate, organized, and able to work in a fast-paced setting with complex barriers.
