Registered Nurse Case Manager
Location: Fort Walton Beach, FL
Assignment Duration: This is a 13-week contract position.
Schedule: 8:00 AM - 4:30 PM, every other weekend required.
Job Description: We are seeking an experienced Registered Nurse Case Manager to join our team. This role involves providing acute care case management to patients within a hospital setting. The ideal candidate will be knowledgeable in Meditech, Midas, and Interqual.
Responsibilities:
- Manage patient care effectively during hospital admission and discharge.
- Conduct assessments and develop care plans based on patient needs.
- Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
- Utilize clinical knowledge and resources effectively for decision-making.
- Facilitate communication between patients, families, and healthcare professionals.
Required Experience / Certifications / Licensure:
- Acute (hospital) Case Management experience required.
- Valid RN license in the state of practice.
- Competency in using Meditech, Midas, and Interqual.
