Ensure effective and efficient operation of the Pharmacy, assures the pharmacy is providing optimal patient care while maintaining the necessary regulatory standards. As Pharmacist-In-Charge (PIC) provides Pharmaceutical Care to patients with an emphasis on clinical monitoring and therapeutic interventions that improve patient outcome. Assist with the maintenance of Performance Improvement processes to constantly improve our Medication Use Process. Assure dispensing and restocking accuracy and direct pharmacy workflow to assure accurate and timely dispensing and distribution. Demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background. Job Description:

- Supervise the day-to-day operations and clinical staffing of pharmacy.
- Reviews /interprets medication orders from licensed prescribers or their agents, ensuring that they meet the requirements of a complete medication order and are appropriate for the patient's condition.
- Maintains familiarity with all departmental programs and technologies and works as an integral part of the pharmacy team.
- Oversees and evaluates ongoing reporting and analysis of existing projects and new initiatives and implements policies and procedures of the Pharmacy Department.
- Clinical Practice and Oversight: Actively participates in the ongoing monitoring of medication therapy in compliance with Pharmacy and therapeutics, Infection Control, and medication management protocols, policies, and procedures.
- Documents medication therapy assessments and recommendations in the patient's chart using iVents and progress notes to keep the interdisciplinary team appraised of pharmacy related assessments and interventions.
- Works with the pharmacy leadership team to implement medication management decisions.
- Assists in achievement of the department commitment to be innovative, proactive, and accountable for quality patient outcomes by optimizing medication use across the continuum of care.
- Compounds and Dispenses Pharmaceuticals: Insures timely dispensing of appropriate quantities of the correct medication.
- Supervises compounding according to USP standards and verification of ingredient compatibility, content, and appropriate labeling including beyond-use dating.
- Checks all medication prior to final dispensing for direct patient administration, distribution via automated dispensing cabinets and floor stock dispensing.
- Uses barcode technology when possible to insure accurate dispensing.
- Supervises the repackaging of medication including verification and documentation of lot numbers, and beyond-use dates.
- Supervises and Directs Technician: Provides active direct supervision and mentoring Technicians as guided by ICARE values.
- This includes monitoring timely completion of tasks, redirection of staff to meet patient needs and encouraging use of the AIDET model in interaction with staff, patients and visitors.
- Reports behavior not responsive to direct intervention to the leadership team.
- Medication Use Safety Initiatives: Detects and reports in a timely manner variances in dispensing on internal variance report forms or in Midas as is appropriate.
- Identifies opportunities and participates in departmental efforts to increase the safe use of medications (e.g. clear labeling, new sound-alike-look-alike medications and Epic improvements / corrections).
- Identifies and reports suspected adverse drug reactions in Midas.
- Controlled Substance Accountability - Participates in and supervises the receipt, dispensing and distribution of controlled substances through the use of Pyxis C-II Safe, Pyxis ES Medstation, internal perpetual inventories, Pyxis Anesthesia Stations, Anesthesia carts and controlled substance waste logs.
- Ensures inventories are accurate, reconciles controlled substance waste and participates in the resolution of controlled substance inventory discrepancies including provision of Pyxis activity reports to nursing.
- Reconciles the Pyxis vs. CII Safe report to insure the appropriate disposition of controlled substances to Pyxis Medstations.
- Contributes to the Positive Overall Financial Status of the Department: Encourages prescriber compliance with formulary restrictions and therapeutic interchanges.
- Assures formulary adherence and processes non-formulary requests in the appropriate manner.
- Suggests appropriate therapeutic alternatives for non-formulary requests.
- Actively participates in defined IV to PO conversions. Makes efficient use of time, hands off work when appropriate to minimize overtime.
- Drug Information / Staff Development: Provides accurate, adequate and timely drug information to the medical staff, nursing staff, and patients.
- Serves as a liaison to Nursing and the Medical Staff and works directly with these groups to meet the needs of the department and the hospital.
- Participates on multidisciplinary committees at the discretion of manager.
- Provides leadership and serves as a role model and resource person for pharmacy staff.
- Coordinates professional growth and development of staff through educational programs, in-services, mentoring, and testing.
- Assures competency through education, coaching, auditing, and testing.
- Coordinates efforts with other St. Elizabeth Healthcare facilities.
- Maintains Professional Competency: Maintains licensure. Completes all departmental competency/skills assessment requirements.
- Maintains current knowledge of pharmacy practice. Seeks to develop and maintain skills in areas of specialized practice.
- Accepts Responsibility of Pharmacist-In-Charge: Responsible for the operation of a pharmacy in conformance with all laws and pertinent administrative regulations including but not limited to those enacted by the State Board of Pharmacy, Joint Commission, DEA, FDA, EPA, State Health Departments, and/or other agencies providing oversight of healthcare delivery.
- Is personally in full and actual charge of the pharmacy.
- Graduate from ACPE accredited pharmacy college.
- Current Pharmacist license in applicable state of practice.
