If you want to make a difference, provide meaningful one-on-one care, and develop relationships with both your patients and team members, join Team 3HC! 3HC strives to provide a positive work culture that feels more like a family atmosphere where we support our team members so they can reach their personal & professional best.Summary:The Hospice Nurse, RN is responsible for the coordination of all Hospice services in counties assigned in accordance with the Hospice philosophy, Agency goals, objectives, and policies.Essential Job Functions:Abides by and supports 3HC's Compliance Program and Code of Ethics.Creates positive experiences for internal and external customers that will meet their expectations.Provides comfort to patient/family to facilitate quality of life and death with dignity.Maintains current electronic documentation and downloads daily.Ensures that the physician and/or Hospice Medical Director is involved ongoing with the patient's care.Provides regular supervision to the Licensed Practical Nurses and paraprofessionals.Collaborates with supervisor to meet assigned productivity standard.Maintains regular communication with supervisor.Ensures that 3HC can bill for services appropriately.Stays abreast of overall patient care and improves nursing care skills.Notify attending physicians, staff members and family members of patient deaths when they occur.Collaborates with Procare Pharmacy regarding medication changes, administration, and recommendations.Demonstrates an interest in personal and professional growth for self and staff.Demonstrates a willingness to be cost effective in the use of agency resources, the monitoring of waste, and the proper and safe use of supplies and equipment.Adheres to 3HC's Personnel Policy and performs other appropriate duties as assigned by supervisor to promote the successful operations and future growth of 3HC.Requirements:Registered Nurse currently licensed to practice nursing in the state of North CarolinaMinimum of one-year experience as a professional nurse in an acute care settingEvidence of leadership qualitiesPrevious experience working with terminally ill patients preferredAbility to understand and communicate well with patients, family, staff, physicians, and the publicMaintain current CPR certification (BCLS)Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the publicMust be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient's individualized a plan of carePossess valid Driver's License and reliable transportation3HC is an equal opportunity employer.
