Anchor Health is seeking a Registered Nurse and Licensed Vocational Nurse to serve in the San Mateo County./ppThe Registered Nurse will plan, organize and direct hospice care. They will also build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities./ppPosition Responsibilities:/pulliCoordinating the care of the patients assigned to you./liliMaking regular visits to patients homes to assess patients physical, emotional, spiritual, and social needs./liliWorking collaboratively with the hospice physician and your supporting LVNs to carry out appropriate interventions./liliHelping the agency to continuously increase the quality of care./li/ulpPosition Requirements:/pulliLicensed as a Registered Nurse/RN./liliValid Drivers License and reliable vehicle with current insurance./liliMinimum of two years experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred./liliCPR Certified./li/ulpHospice LVN Duties Include:/pulliService a range of patients and family during transition of life./liliEnsure quality and safe delivery of Hospice services./liliImplements current nursing practices and provide direct hands-on care within scope of practice./liliObtains orders as needed to promote comfort and quality of life./liliKeep Case Manager informed of all changes in condition and orders obtained./li/ulpJob Types: PRN, Per diem/ppMedical Specialty: Hospice Palliative Medicine/ppPhysical Setting: Inpatient, Outpatient/p/div
