Youll visit Medicare Advantage plan members across Ector County, including West Odessa and Goldsmith, performing evaluations crucial to preventive care.

Key Responsibilities:

• Conduct comprehensive annual risk assessments during 45-minute home visits to assess patient history, current conditions, and medication use.
• Educate members about medical conditions and their medications to support ongoing health.
• Identify gaps in care and offer professional recommendations to help close those gaps.
• Focus exclusively on assessment and educationno prescribing or treatment planning involved.

Schedule:

• Flexible shifts Monday through Sunday
• Self-scheduled hours to support your work-life balance
• Minimum weekly or bi-weekly hourly commitment required

Qualifications:

• Active Nurse Practitioner license in Texas
• Board Certification from AANP or ANCC
• Current BLS (CPR) certification
• MSN from an accredited nursing program
• At least six months of nurse practitioner experience is preferred
