Physical Therapist

BoldAge PACE seeks a compassionate Physical Therapist to deliver person-centered care to older adults in our coordinated healthcare program. In this full-time role, you will assess mobility, strength, and balance; create individualized, evidence-based treatment plans; and provide therapeutic interventions to help seniors maintain independence and quality of life across home, clinic, and community settings. You’ll collaborate closely with an interdisciplinary team, document in our EMR, educate participants and caregivers, and contribute to a supportive, learning-focused culture that values teamwork and professional growth.
