Responsibilities: Provide customized care and support to clients in their homes, focusing on their physical, emotional, and social well-being Assist with daily living activities, including personal care, medication management, and light housekeeping Develop and maintain strong relationships with clients and their families, promoting open communication and trust Collaborate with other healthcare professionals to ensure seamless care transitions and continuity of services Maintain accurate and timely records of client care and services provided Requirements: 1+ years of experience in a healthcare setting, preferably in home care CNA preferred but not required Ability to lift, push, or pull up to 50 pounds and assist with transfers and ambulation Reliable transportation and a valid driver's license High school diploma or equivalent required; certification in CPR and First Aid a plus A dynamic and supportive work environment with opportunities for professional growth About Us: At SYNERGY HomeCare, we believe that our purpose is to provide the best care possible.
